Behavioral Health Care Manager

NEW Health is a federally qualified health center (FQHC) that is expanding its primary care setting to include behavioral health and will be engaging in the Collaborative Care model to support the integration of behavioral health into its primary care settings at its Colville and Chewelah clinic locations. NEW Health will be hiring a behavioral health care manager for each location who will be responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and when appropriate other mental health providers.

The behavioral health care manger will be an integrate piece in NEW Health’s behavioral health care team. The behavioral health care manager will: screen, access, facilitate and educate, and track common mental health conditions and facilitate care. They will also provide brief behavioral interventions using evidence-based techniques and support psychotropic medication management, as prescribed my medical providers. The behavioral health care manager will be a part of an integrated care team, including the primary care physician and a psychiatric consultant.

 

NEW Health requires its behavioral health care managers to have the following educational requirements: Licensed Mental Health Counselor/Professional Counselor, Licensed Social Worker, Registered Nurse (BSN required) with behavioral health experience, or Masters-level licensure candidate/trainee (e.g. LMSW) required. Experience screening for common mental health and/or substance abuse disorders preferred. Knowledge of differential diagnosis of common mental health and/or substance abuse disorders preferred. Knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders preferred. The right candidate will demonstrate an ability to collaborate and communicate effectively in a team setting and have the ability to work with patients by telephone as well as in person.
